W przypadku zagubienia legitymacji studenckiej należy zgłosić ten fakt do dziekanatu i złożyć wniosek o duplikat, wydrukowany z USOS

You have to pay for the duplicate and cost is 33 zlotys.

Za duplikat trzeba zapłacić, a koszt to 33 zł.

I would like to pay on installments. Do I have to report this?

Chciałbym zapłacić na raty. Czy muszę to zgłosić?

Yes, I’m affraid you do. To pay for a semester in installments you have to submit an application to the dean’s office to divide the payment into installments.

Payment in installments is 5% higher then on-time payment.
